1. Services
We provide non-medical home care and personal assistance services. Services are scheduled based on client needs and availability. We reserve the right to refuse or discontinue services for safety, non-payment, or policy violations.
2. All service payments are non-refundable. Payments are due as agreed in your service plan or invoice.

A $100 late fee applies after the third day of non-payment.

An additional $25 per day will accrue until payment is received.

Services will be suspended after 7 days of non-payment, and legal action may be taken to recover unpaid balances.
3. Cancellations
Cancellations must be made at least 24 hours in advance. Late or same-day cancellations may result in a full charge.
4. Liability Disclaimer
While we provide care with professionalism and compassion, Azaria’s Home Assistant Agency and its staff are not responsible for injuries, accidents, or damages caused by unsafe conditions or client negligence. Our services are non-medical and do not replace medical advice or treatment.

6. Termination of Services
Services may be terminated for non-payment, violation of policies, unsafe conditions, or other reasons deemed necessary by management.
